What is a cough?

Coughing is our body’s defense reflex in response to an irritation in the throat or airways, which tries to expel the irritant agent to the outside. In other words, it’s a defense mechanism through which our body “cleanses” the lungs and airways. Since coughing is a protective mechanism, it is not advisable to suppress it.

It’s important to know how to distinguish between the different types of cough, as this symptom can shed light on the health problem that is causing it and thus guide appropriate treatment. Although there are some diseases that can be associated with both a dry cough and a productive cough (with expectoration), it is possible to make this distinction.

Dry cough

Unlike a productive cough, a dry cough does not produce any kind of fluid/mucus and most often develops in response to irritation or inflammation of the airways.

Among the main causes are: asthma, pulmonary fibrosis, allergies, gastroesophageal reflux, certain medications (such as for blood pressure), exposure to irritants (dust, smoke or chemicals), blockage of the airways by a foreign body (food, pills, etc.), among others.

Treating the underlying cause is often the best strategy for reducing the severity and frequency of dry cough. However, there are some treatments and measures that can improve a dry cough if it is associated with a definite cause and for short, non-severe periods, such as throat lozenges, cough suppressants (which can reduce the cough reflex) or simply raising the head of the bed.

Productive cough

A productive cough, or cough with sputum, occurs when, associated with the cough reflex, there is production of fluid or mucus in the airways – in the throat, nose or bronchi and lungs.

Among the main causes are: smoking, chronic lung diseases (such as chronic obstructive pulmonary disease and asthma), heart problems such as heart failure (fluid mucus without signs of infection) or respiratory infections (such as colds, bronchitis and pneumonia).

When a productive cough is associated with a respiratory infection, other symptoms can also occur, such as a sore throat, fatigue, fever, shortness of breath and wheezing (a whistling sound produced during breathing).

The treatment of productive coughs aims to help expel mucus and cleanse the airways. This is the case, for example, with expectorants and mucolytics, which make the mucus more fluid, facilitating its expulsion through coughing. On the other hand, suppressing a productive cough can lead to the accumulation of mucus and, consequently, other complications such as breathing difficulties and infection. If coughing up sputum is associated with another health problem or signs such as fever, treatment may include taking other medications, such as antibiotics prescribed by the doctor.

What to do at home, as a first approach

To speed up the disappearance of the cough, there are small gestures you can put into practice such as resting well and with your head elevated, staying hydrated, making vapors, eliminating irritants from your home, quitting smoking and avoiding smoky environments.

When should you see a doctor?

In the case of children, you should seek medical advice whenever you observe the following symptoms:

  • Sudden onset cough (may be due, for example, to inhaling a foreign body);
  • Cough lasting longer than expected (around 3-5 days) whether or not it is associated with a cold;
  • Cough that interferes with sleep quality or daily life;
  • Associated fever;
  • Drowsiness, fatigue and feeling unwell;
  • Difficulty breathing, wheezing (“kittens”).

For adults, medical advice applies to the following symptoms:

  • Sudden shortness of breath, chest pain or tiredness;
  • Coughing up blood or mucus;
  • Cough that interferes with sleep;
  • Other associated problems for no apparent reason, such as weight loss or fever;
  • Swelling in the legs, feet or ankles.

It is important to note that, in addition to these points, any situation that causes significant concern or discomfort should be a reason to seek medical advice.

A health professional will be able to carry out a full assessment and, if necessary, request additional tests to determine the cause of the cough and the most appropriate treatment.
